OFFICE APPOINTMENTS 

1. You are required to wear a mask in our office at all times.

2. Please come into the office no earlier than 15 minutes prior to the start of your scheduled                          appointment.

3. Office staff will check your temperature and ask you some Covid-19 related screening questions upon arrival.

4. All fees must be paid in full before seeing your provider.

*IF YOU ARE EXPERIENCING ANY OF THE SYMPTOMS BELOW, DO NOT COME INTO THE OFFICE.

GIVE US A CALL AND WE CAN EITHER CHANGE YOUR VISIT TO TELEMEDICINE, OR WE CAN RESCHEDULE YOUR APPOINTMENT.

  • Fever or chills

  • Cough

  • Shortness of breath or difficulty breathing

  • Fatigue

  • Muscle or body aches

  • Headache

  • New loss of taste or smell

  • Sore throat

  • Congestion or runny nose

  • Nausea or vomiting

  • Diarrhea

 TELEMEDICINE APPOINTMENTS​

(1.) Call our office at 503-266-1407, thirty minutes prior to the start of your appointment.

(2.) Supply payment information to staff and inform them that you are ready for your appt.

*All copays, coinsurance, deductibles and self-pay amounts are due before meeting with your provider.

(3.) Click on your provider's picture after selecting the "Patients" tab at the top of this page.

After clicking on their picture, you will enter into the Zoom meeting waiting room where your provider 

will meet you when they're ready. 

 

Call the office again IF........

  • The provider does not connect within 3 minutes after your appointment time should start.

  • You are disconnected during the session. (Please try to reconnect twice before calling back)
